Position Overview:
We are seeking a dedicated and skilled Behavioral Health Counselor (LGSW or LGPC) to join our Southeast DC team. The counselor will provide assessments, counseling, and case management as part of a multidisciplinary care team. This hybrid role involves both in-person field visits and remote work.

Key Responsibilities:
- Conduct biopsychosocial assessments and develop individualized care plans.
- Provide therapeutic counseling and supportive case management.
- Collaborate with healthcare professionals, caregivers, and community resources.
- Maintain accurate and timely documentation in compliance with agency and regulatory standards.
- Participate in interdisciplinary case reviews and team meetings.
Qualifications:
- Active DC LGSW or LGPC license required.
- Minimum 1 year of experience in behavioral health, community-based care, or related field.
- Strong interpersonal, organizational, and communication skills.
- Ability to work independently and collaboratively within a team.
- Commitment to trauma-informed, patient-centered care.
